The Northeast Florida Healthcare Coalition has allocated money to fund projects for its member organizations. Interested members must complete the Project Submission Form below.
The application must be completed in its entirety; applicants must submit all requested information. Incomplete applications and applications that do not meet the requested standards may be rejected, or be at a disadvantage, in comparison with those that are complete.
The application period will be open throughout the fiscal year (ending June 30, 2018). Projects will be funded based on a capability based priority and funding availability. Please submit completed applications to Beth Payne, email@example.com.
The Northeast Florida Healthcare Coalition’s Project Funding Guidelines include:
Requesting agency must be a current member or request membership (via online form) to the Northeast Florida Healthcare Coalition as part of the project submission process.
Request must demonstrate relevance to the Coalition’s mission: To achieve and health and medical system that is efficient and resilient in an emergency.
Projects will provide for geographic diversity within the Coalition’s six county region of Baker, Clay, Duval, Flagler, Nassau and St. Johns Counties.
Projects must address an identified healthcare delivery deficiency, capability or resource gap.
Projects will not be considered if they supplant normal business expenses/core mission requirements
Projects will not be considered if they violate any of the ASPR funding restrictions
Decisions made on funding requests are at the sole discretion of the Northeast Florida Healthcare Coalition Executive Board.
The Northeast Florida Healthcare Coalition Executive Board reserves the right to partially fund a request.
