Strategic Plan
NEFLHCC Multi-Year Strategic Plan (2014-2017)

The Strategic Plan brings together the various components of the NEFLHCC’s founding tasks and efforts, and constructs a snapshot of the organization’s current capabilities, resources and gaps. This document then informs the Coalition’s future activities and initiatives. This plan is intended to be revised and built upon in future years, as the organization expands and matures, and the goals and objectives adapted to changing preparedness and response requirements and environments.

This plan also details the information sharing role of the Coalition.

The Northeast Florida Healthcare Coalition Risk Assessment identifies potential threats, the probability of occurrence  and document the community’s contingencies plans should the incident occur . Essential healthcare services are included as well as an analysis of the community’s response and resource capabilities related to the threat. This is updated annually.

The Northeast Florida Healthcare Coalition (NEFLHCC) Continuity of Healthcare Operations Plan (CONHOP) is consistent with the overarching Strategic Plan, and is an annex to the Coalition’s operational coordination plans developed by the Coalition. The CONHOP addresses short- and long-term support of essential and prioritized healthcare assets in the six-county region of Baker, Clay, Duval, Flagler, Nassau and St. Johns counties, and outlines the general processes to support the continuation of healthcare Mission Essential Functions in this region.

The Northeast Florida Healthcare Coalition (NEFLHCC) Patient Tracking Monitoring Plan outlines how patients are tracked by the member organizations within the six county region of Baker, Clay, Duval, Flagler, Nassau and St. Johns counties. The scope of this process is to document the existing plans and procedures in place to track patients by the counties and hospitals within the Northeast Florida Healthcare Coalition. These plans, policies and procedures are relevant to the member agencies of the Northeast Florida Healthcare Coalition, as used during a public health event or other emergency, threat, or impact to the health and medical system in the NEFLHCC region.

This document also details the State of Florida resource request policy. The scope of this process is to document the existing plans and procedures in place by the counties and agencies within the Northeast Florida Healthcare Coalition agency.
